  • The Sports Medicine Department Staff would like to welcome all new and returning student-athletes to West Virginia State University.
  • Our first priority is ALWAYS the student-athletes overall safety and medical needs. The Athletic Training Room is located on the first floor of Fleming Hall (N130).
  • You must keep the WVSU Athletic Training Staff informed anytime you are injured or ill and prior to any Doctor related visit. The Student-Athlete has the direct responsibility for reporting all of your injuries and illnesses to the sports medicine staff at WVSU. Failure to communicate can result in the loss of practices/games.
  • The Athletic Training staff at West Virginia State University is dedicated to treating each student-athlete on an individual basis with an emphasis on injury prevention and safe return to participation.
  • Services provided include prevention, evaluation, injury management, treatment and rehabilitation of injuries sustained as a direct result of participating in practices and/or games.

Important Notice

 

·       FAXES WILL NOT BE ACCEPTED

 

·       THE NECESSARY STUDENT-ATHLETE FORMS TO PRACTICE/CONDITION ARE PHYSICAL, PROOF OF INSURANCE, AND SICKLE CELL TEST PER NCAA.

 

·        HOWEVER WVSU SPORTS MEDICINE REQUIRES ALL PAPERWORK MUST BE COMPLETED TO PRACTICE/CONDITION.

 

·       ALL STUDENT ATHLETE INFORMATION WILL BE UPLOADED ONTO THE HEALTHY ROSTER PLATFORM

 

·       Athletes will not be allowed to participate in any WVSU Sports Related activities until all Medical Paperwork has been completed and appropriate files have been uploaded. The medical paperwork is reviewed by the Sports Medicine Staff. 

 

New Freshman/ New Transfer Student Athlete Healthy Roster instructions



1)    Each New Student-Athlete/ New Transfer will submit to their coaches:

a.     First and Last Name

b.     Email

c.     Phone Number

d.     Birthday

e.     Sex (Male or Female)

f.      Cell Phone

g.     Address

h.     1 Emergency Contact

 

2)    Student Athlete will download Healthy Roster application on their phone (They can also access Healthy Roster by Web at https://www.healthyroster.com/)

 

3)    Student Athletes will receive a 6-digit invite code, to the email they submitted to the coaching staff, from healthy roster. That code will be used to log-in and set-up student athletes profiles.

 

4)    Once a Student Athlete has set up their profile they will click on documents:

a.     The following documents only require an electronic signature and date. They include:

                                               i.     Alcohol and Drug Policy

                                             ii.     Assumption of Risk and Consent to Treat

                                            iii.     Confirmation of Concussion Baseline

                                            iv.     HIPPA Form

                                             v.     Injury and Illness Form

5)    The next set of documents require you to add multiple answers and/or signatures and dates, or require you to upload the appropriate files:

 

a.     Physical Form

                                               i.     (Use WVSU Physical Form, Can be downloaded from Healthy Roster Under Physical Form Tab)

                                             ii.     They cannot use a High School Physical Form

b.     Sickle Cell

                                               i.     Joining a Sport Team for 1st time after Fall 2022 - you must have a Hemoglobin Solubility test for Sickle Cell or you cannot practice/play. This is the same rule as NCAA D1. LAB Corp will do this for a discounted rate for athletes in Charleston if they need one.

c.     Medical Insurance Card

                                               i.     Front and Back Image of the Card

d.     Mental Health Questionnaire

e.     Medical History Form

f.      Insurance and Emergency Contact



·       Student-Athletes will then receive and separate email from Impact Test Online for them to complete their Baseline Concussion Test Electronically. 

·       The email that will be from Impact Test and will give instructions and the test code for the student-athlete to use. (PLEASE USE INTERNET EXPLORER OR CHROME TO COMPLETE THIS TASK)

·       Make sure to complete all information and concussion history on page 1 (DO NOT SKIP)

 

·       Until the Student-Athlete has been given confirmation that they have been cleared by the Sports Medicine staff prior to the start of their 1st practice the Student Athlete should assume they are missing paperwork or it was not completed in its entirety. The student-athlete needs to check periodically that all appropriate paperwork has been approved for participation. 

 

·       The student athlete will continue to use healthy roster in the case of an injury. They will upload pertinent medical documents, visits, follow-ups, etc. for all visits to Marshall Orthopedics or visits that are sports related at WVSU in the case that be sent home to receive care. 

 

 

MENTAL HEALTH 

·       All New Student Athletes must complete the Attached mental health form Per NCAA. This form gives a basic starting point for the Athletic Training Staff
